| Lab-Wide Subcontractor Job Hazards Analyses and Work Authorizations (SJHAWA)
In order to reduce redundant safety assurance paperwork, we have begun preparing Lab-wide Subcontractor Job Hazards Analysis and Work Authorizations (SJHAWA) for qualified subcontractors and vendors who are performing scopes of work with low level hazards. The Program Manager performs the role of the "Requester" for Lab-wide SJHAWAs and he administers completion of the SJHAWA forms and the general pre-job meetings. The completed Lab-wide SJHAWAs are posted on this web page with unique identification numbers so that "end-users" can reference them when they submit requisitions for services. Lab-wide SJHAWAs still require each of the “end-users” of the services to hold a brief pre-job meeting to discuss any specific conditions or areal hazards related to their work, and also provide and record oversight as required. Additional guidance on use of Lab Wide SJHAWAs will be posted on the web page soon.
