On the green signature block (for Requester/Division Approval): Change the block to read from the following “"Requester/Division Approval:” to “Requester/Division Approval:  I authorize the work to proceed subject to the specified controls."

These changes are required in order for the Chapter 31 work authorization process to achieve equivalence with the institutional JHA requirements in PUB-3000 Chapter 32.

32 — Job Hazards Analysis

Updated chapter based upon lessons learned during the JHA Pilot Program. There are no substantive policy issues raised in this update. Changes include:

  • Reworded implementation schedule by removing references to FY 2007 which has passed.
  • Clarified that Worker’s signature, although desired, is not required. Work Lead’s signature verifies that the JHA has been discussed with the Worker, that readiness review has been conducted, and that the Work is authorized.
  • Pulled out Appendix C as separate, downloadable document; revised the signature blocks on these documents to reflect implications noted above re: Worker and Work Lead signature.
  • Clarified that JHA requirements for construction are given in PUB-3K, Chapter 10.
  • Refined descriptions of typical tasks, hazards and controls as used in the JHA.

Clarified that the Individual Baseline JHA is accessed through the automated JHQ software, and that the Task-based JHA is a paper system not accessed through a central system.
