Who needs to know about these requirements?

The requirements of this PUB-3000 chapter, sJHA Process — Subcontractor Job Hazards Analysis, apply to:

• Berkeley Lab requesters of nonconstruction services performed by subcontractors, vendors, and affiliates, as defined in Section 31.5, Roles and Responsibillities


• Berkeley Lab subcontractors, vendors, and affiliates who perform hands-on work at facilities owned or leased by the Laboratory
  
  

Why?

• Subcontractors who perform hands-on work at Berkeley Lab are required to submit safety assurance documents appropriate for their scope of work. These documents typically include scope of work, Job Hazards Analysis, and special permits and work authorizations. Before work can begin, these documents must be approved by Berkeley Lab. During the performance of subcontractor work, Berkeley Lab provides oversight that is commensurate with the work hazards. See the LBNL Worker Safety and Health Program (PUB-3851), Section 1.6, Flow Down of 851 Requirements to Subcontractors.


• Every worker at Berkeley Lab must be either directly supervised or have a current work authorization resulting from a Job Hazards Analysis process, as defined in Chapter 32, Section 32.5, Job Hazards Analysis Process.

What you need to do before your subcontractor, vendor, or affiliate performs hands-on work

• A Subcontractor Job Hazards Analysis and Work Authorization (sJHA) form must be completed, and the subcontractor, vendor, or affiliate must review the Berkeley Lab ES&H Orientation for Non-Construction Subcontractors, Vendors & Affiliates.
• A pre-job meeting must occur between the requester and the subcontractor, vendor, or affiliate at which:
• The sJHA and any other required work authorizations are reviewed and signed.
• The ES&H Orientation for Non-Construction Subcontractors, Vendors & Affiliates is discussed.

During the conduct of the hands-on work performed by the subcontractor, vendor, or affiliate:

• The requester provides oversight of the subcontractors using a risk-based graded approach (See Work Process A, sJHA Process Overview).
