PUB-3000 18.5.1 Equipment-Specific Written Energy Control Procedure
An equipment-specific written energy control procedure must be developed and used whenever equipment or apparatus undergoing servicing, modification, or maintenance:

Describe equipment to be worked on, applicable tasks as necessary, energy hazards to be controlled, etc |

Preparation and Notifications |
Identify persons to be notified |

Equipment Shutdown |
Specify all steps of how to safely shut the equipment down, assuming it is running. |

Energy Isolation |
Identify all of the types and magnitudes of hazardous energy |

Device application |
Identify energy isolation points and methods for all hazardous energies to be controlled. Note: “Challenge the lock” |
Identify any special LOTO devices to use for all hazardous energies to be controlled. |

Additional Measures |
Describe how to release stored energy and prevent reaccumulation of hazardous energy. “Bleed, block, and restrain” |

VERIFICATION!! |
Instructions on how to attempt to restart equipment to check effectiveness of LOTO Identify any tests or measurements necessary to verify absence of energy, including special instructions, e.g., qualified person, etc. |

Release from LOTO |
Provide detailed step by step instructions of the restart sequence: Device removal, repositioning of controls, replacement of covers, tests, notifications, etc. |
