1.11.1 APPENDIX A — PROCESS FOR REVISING THE LBNL HEALTH & SAFETY MANUAL (PUB-3000)

1. MAJOR CHANGES

Changes to Chapter content that add new policy, procedures, guidance, and/or requirements that have a significant impact on mission/operations.

1.1 Drafts final version of all changes (changes may be requested by anyone).

1.2 Submits to EH&S Chapter Technical Reviewer for concurrence that the revision is necessary and technically sound.

EH&S PUB-3000 Manager

2.1 Analyzes effect of proposed changes on other PUB-3000 chapters.

2.2 Coordinates institutional review with the following:

  • EH&S Work Smart Standards Update Coordinator
  • Human Resources (if applicable)
  • EH&S Director (or designee) for final approval

2.3 Presents revision to Safety Review Committee for endorsement.

3.1 Endorses changes.

4.

EH&S PUB-3000 Manager

4.1 Forwards changes endorsed by Safety Review Committee to Technical & Electronic Information Department (TEID).

4.2 Briefs EH&S Coordinators.

4.3 Notifies Lab employees by Policy & Procedure Memo (P&P via e-mail with hard copy to Division EH&S Coordinators).

4.4 Submits archival records to TEID, documenting changes.

5.

TEID Editor

Edits proposed changes, publishes changes on Web (after final review by EH&S PUB-3000 Mgr.) and maintains archival records.

2. MINOR CHANGES

Technical changes that are necessary to reflect regulatory (WSS set) changes, best practices, etc. These changes have minimal effects on policy, e.g., adding a compressed gas to the NFPA Health Hazard Class 3 list (as a result of an NFPA change).

Changes that have no effect on policy, e.g., format, numbering systems, syntax, etc.

1.1 Drafts changes, confirms that the changes have no effect on policy, and establishes that changes are necessary (changes may be initiated by anyone).

1.2 Approves changes.

1.3 Ensures that documentation is archived.
