Potential Maintenance and Operations Questions

Division Directors

  1. Describe your expectations for safety, how you communicate these expectations to your organization, and how you know the expectations are understood and owned by staff.
  2. Who is responsible for the safety of maintenance and operations activities?
  3. Describe how your organization identifies and analyzes hazards for its work and determines appropriate controls. 
  4. Tell me about the role of the line managers, workers, Safety Coordinator, the Safety Liaison, and the supervisors in this process.
  5. Describe for me how work is authorized, and who authorizes the work.
  6. Describe how it is assured that the staff understand the hazards and use the prescribed controls.
  7. How frequently are you in the work area?  How do you observe staff performing their work?
  8. Describe what you expect the supervisor to do in assuring safety of work activities and tasks/jobs.
  9. As tasks evolve, tell me how any changes in the scope of or hazards associated with the work are identified and examined to determine if changes in controls are needed.
  10. How do you evaluate the overall safety performance of your organization?

 

Supervisors

  1. Describe your role relative the safety on the job?
  2. Tell me about the tasks and activities being conducted by this organization.
  3. Describe the hazards associated with the type of work your organization does, the hazards analysis that was conducted, and who was involved in the hazards analysis (e.g., crafts personnel, Subject Matter Experts/Safety Liaisons).
  4. What are the hazard controls that are in place?  How do the controls mitigate or eliminate the hazards?
  5. Describe how information regarding hazards and controls is communicated to craft personnel and/or what training has been provided to the staff for these controls.
  6. Describe how you satisfy yourself that the staff understand the hazards and use the prescribed controls.
  7. What is done to authorize the start of a task or activity?  What role do you play in this authorization?
  8. How frequently do you observe work activities under your authority?  Tell me how you observe staff performing their work.
  9. How are any changes in the scope of a job/task or hazards associated with tasks or activities identified and examined to determine if changes in controls are needed?
  10. Who is responsible for the safety of the work?
  11. Describe how you assure that all the personnel or groups working in an area are aware of adjacent hazards and controls.  How are staff qualified to work on the various tasks?  What is meant by “skill of the craft”?  Under what kinds of conditions is “skill of the craft” sufficient to perform a task?
  12. What action would you take if you see an unsafe condition or unsafe behavior on the part of someone under your supervision in the workplace?  Can you give me an example?

Craft Personnel

  1. Tell me about your work and the types of hazards that exist (on this task/job).
  2. What type of analysis or review was done to identify the hazards associated with this task/job.  Who was involved in the hazards analysis (e.g., Subject Matter Experts/Safety Liaisons)?  Were you asked for input?
  3. What hazard controls are in place?  How do the controls mitigate or eliminate the hazards?
  4. What sort of training or qualification do you have regarding the hazard controls?
  5. Do you use any procedures to conduct your work activities?  Are there any permits in place associated with this work?  Where are those procedures/permits?  Do you review the procedures before starting work? 
  6. What do you do when during the course of a job, the conditions change or something unexpected is encountered?  What happens if the procedure steps don’t make sense while you are conducting the work?
  7. Who is in charge of the work activity and the safety of it?  What is your role in the safety of the job?
  8. What actions might you take personally if you see an unsafe condition or unsafe behavior on the part of someone else on a job you are involved in?
