Nomination Form

To nominate a co-worker for a Safety Sport Award:

1. Complete the nomination award form, and
2. Send the form to safetyspotaward@lbl.gov

The Safety Spot Award Committee will review the form and reply to you within 48 hours. Approved awards will be processed by HR & Payroll, and certificates will be signed by EH&S Division management and sent to the nominator.
