Catastrophic Leave Process

1. Employee submits "Catastrophic Leave Request Form" to the Disability Specialist in Health Services. If the employee is unable to submit their own request, family members or potential leave donors should contact Health Services directly. (510)486-5213. 2. The Disability Specialist determines eligibility. 3. If eligible, the disability Specialist works with the employee and Payroll to determine how much time will be needed under the catastrophic leave sharing program. 4. Disability Specialist assigns a case number for reference purposes. 5. The Disability Specialist notifies Payroll, Labor/Employee Relations, and the HR Center that an employee is eligible to receive donations. 6. The Disability Specialist will work with the HR Center and/or employee to determine how to notify potential donors. If the employee does not wish to publicize their name, the case number (from step #4) should be used as reference. 7. People who wish to donate time must download the "Catastrophic Leave Donation Form" and send to the Disability Specialist at mailstop 26-143. 8. The Disability Specialist will collect donation forms and add the recipient name (if necessary). 9. The Disability Specialist will forward all donation forms to the Payroll Department. 10. Payroll will process all forms on a first received, first used basis. The Payroll department will notify donors if their time was not used. 11. If the employee needs to extend catastrophic leave time, they must coordinate with the Disability Specialist to determine eligibility and repeat steps #2, through the end of the process. 12. Once the employee is ready to return to work, they must contact the Disability Specialist for a release to return. 13. The Disability Specialist will notify the HR Center of the employee's return to work. 14. The HR Center will notify Payroll of the employee's return from leave. 15. Payroll will return the employee from leave in the HR Database and will notify donors of any unused time.
