ePro FAQ How do I Check the ePro Services Box?

Berkeley Lab subcontractors (vendors) are required to submit a written Worker Safety and Health Plan (WSHP) to LBNL to protect their workers at LBNL and DOE sites. Further information is in the Guide for On-Site Subcontractor Safety Plans. (See below for pilot divisions.) A Services drop-down box appears on the ePro Requisition Summary page when a requisition has an item whose Item Category could potentially involve on-site services. This lets Procurement know when the work involves on-site services with hands-on work so the applicable safety clauses and forms can be sent to the subcontractor. When the Services box appears, it is initially blank. You must change its value to one of the following four choices before you can Save the requisition:

On-Site services at LBNL are considered to be work performed on the hill or at these LBNL Offsite, Leased Facilities. The Subcontractor Safety Information link on the ePro requisition directs you to this page.
Pilot Subcontractor Safety Program: For pilot divisions (EH&S & MS), Berkeley Lab non-construction subcontractors (vendors) are required to complete the Subcontractor Job Hazard Analysis (SJHA) form prior to starting on-site hands-on work at LBNL. The form assists LBNL Requesters and Division Safety Coordinators in assuring that subcontractor personnel are working safely at LBNL and DOE sites. The Requester/Division section of this form should be filled-in by the Requester and the form should be attached to the eProcurement requisition. Procurement will then forward the form to the subcontractor to complete and bring to LBNL prior to commencing work. Here is a flowchart of the pilot Non-Construction Subcontractor Safety Process.
