Gym Waiver Form
In order to use the gym in the Leap Frog building , you must first fill out this waiver. Submit the signed waiver to Cyndi, the Parking Garage Manager, who is in the located in the booth in the parking garage. She will make sure it is keyed in to the system. Then you may enter the gym with your cardkey.
